Form Rules & Guidelines

• An Affidavit is required to accompany any petition, motion, or registration involving custody and/or visitation.
• Any form mailed to our office must have the signature properly notarized.
• If you are requesting medical information for a Subpoena Duces Tecum, the Notice to Individual/Patient and Notice to Health Care Entities/Providers must be completed as well.
• Mingled pleadings will not be accepted. In cases with multiple children, each child must have their own motion/petition. custody, visitation, and support matters also must be kept on separate motions/petitions.

Form Fees

There is a $25 filing fee for custody/visitation petitions. There is a $12 Sheriff's Fee for each party to be served for a civil Witness Subpoena, a Subpoena Duces Tecum, a Spousal Support Petition, and a Paternity Petition.

There is an estimated fee of about $100 for each Order of Publication. Since the Court does not charge or collect this fee, please contact the Yorktown Crier at 757-898-7225 for accurate pricing information.
